Ridgeview Health & Rehab Center is looking for an MDS Coordinator to join our growing team!

Are you a Registered Nurse (RN) or LPN searching for a new career opportunity? Ridgeview Health & Rehab is seeking an RN or LPN to join our expanding staff of caregivers at our skilled nursing facility as our MDS Coordinator! You will support our residents and work hand-in-hand with our already wonderful team of compassionate and enthusiastic caregivers.

A primary focus of the MDS is the resident assessment process, which includes individual assessment, MDS assessment, and resident plan of care. The MDS Coordinator manages the resources provided to residents and coordinates the entire continuum of care.

MDS Coordinator's responsibilities include:

  • Provide accurate documentation of all services to assure accurate reimbursement from all payor sources.
  • Ensure that needed resources are available and that quality care is delivered to all residents
  • Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.

MDS Coordinator's Requirements:

  • Must have a current RN or LPN license
  • Must have Long Term Care experience
  • 1 - 2 years MDS experience preferred
  • Must be knowledgeable of long-term care nursing and medical practices and procedures
